Am 19.03.2018 wurde im prestigeträchtigen New England Journal of Medicine (NEJM) die bahnbrechende PRECISION-Studie zum Thema Prostata-MRT veröffentlicht. Die klinische Untersuchung an 500 Männern, die an 25 Kliniken in 8 Ländern, darunter Finnland, England, Argentinien, USA, Italien und Frankreich, durchgeführt wurde, zeigt erstmalig, dass der Einsatz der MRT der Prostata klinisch äußerst wertvoll ist.

Mehr als 400.000 neue Prostatakarzinom-Erkrankungen werden in Europa jährlich diagnostiziert, in Deutschland sind es im Jahr mehr als 60.000 Männer, die diese Diagnose erhalten. Mehr als 10.000 Männer im Jahr sterben an der Erkrankung, und das, obwohl sie bei rechtzeitiger Erkennung gut behandelt und fast immer geheilt werden kann.

Das Standardverfahren für die Diagnose des Prostatakarzinoms ist die transrektale (durch den Enddarm durchgeführte) Biopsie, also eine Entnahme von meist 12-16 Proben aus dem Organ. Neben den Schmerzen, die diese Methode trotz lokaler Betäubung auslösen kann, besteht auch ein geringes Infektionsrisiko.

In der PRECISION-Studie gelang es Forschern nun erstmals zu zeigen, dass mithilfe einer multiparametrischen MRT der Prostata (mpMRT) mehr klinisch relevante Prostatakarzinome entdeckt werden als mit der Standard-Biopsie: bei 38% der Patienten, die zunächst eine mpMRT und dann eine gezielte Biopsie ausschließlich der in der MRT aufgefallenen Areale hatten, wurde Prostatakrebs gefunden. In der Gruppe der Männer, die die Standard-Biopsie ohne vorherige MRT bekamen, waren es nur 26%.

Fast noch wichtiger ist das zweite Hauptergebnis der Studie: 28% aller Patienten, die eine unauffällige mpMRT der Prostata hatten, konnten so eine unnötige Biopsie vermeiden. Auf Europa hochgerechnet, würde dies bedeuten, dass mehr als 250.000 Biopsien jedes Jahr vermieden werden könnten.

In der Studie wurde hervorgehoben, dass nur Radiologen mit ausreichender Erfahrung die Befundung der mpMRT der Prostata vornehmen sollten – in der Studie wurden mindestens 300 MRTs im Jahr gefordert. Ab 03.07.17 läuft unser neues CT – Somatom Definition Flash der Firma Siemens – an unserem Standort im Rotkreuzklinikum München. Dieses Gerät setzt neue Maßstäbe bei der Geschwindigkeit der Untersuchungen und ist zugleich extrem strahlungsarm.

Beispielsweise kann ein Herz-CT nun mit weniger als 1 Milli-Siewert (mSv) durchgeführt werden, bislang waren dafür mehr als 3 mSv erforderlich. Dadurch ergibt sich eine Dosisreduktion um >50%. Aber auch im Bereich aller anderen Untersuchungsregionen wird die Strahlendosis um bis 40% reduziert.

Am 17.05.2017 haben wir unser neues Gerät für die Durchleuchtungsdiagnostik am Standort im Rotkreuzklinikum in Betrieb genommen. Somit steht dieser Bereich der radiologischen Diagnostik ab sofort wieder zur Verfügung.

Das MAGNETOM® Skyra von Siemens ist eine bahnbrechende Weiterentwicklung in der 3Tesla-Bildgebung, das über eine 70-cm-Öffnung für besseren Patientenkomfort und das Tim+Dot-System verfügt.

Tim® (Total imaging matrix) der vierten Generation (4G) bringt die MRT-Bildgebung einen weiteren großen Schritt voran – mit 4G-Flexibilität, 4G-Genauigkeit und 4G-Schnelligkeit. Die brandneue, innovative Technologie von Siemens ermöglicht eine noch nie dagewesene Qualität in der Bildgebung. Das weiterentwickelte Tim bietet eine ultra-hohe Spulendichte kombiniert mit der höchsten Anzahl an Empfangskanälen, die es je gab. Und da die Tim-Technologie sich individuell auf jeden Patienten abstimmen lässt, erreichen Bildqualität und Aufnahmegeschwindigkeit ein völlig neues Niveau.

Mit dem neuen MAGNETOM Skyra werden MRT-Untersuchungen einfacher und bequemer als je zuvor. Die großzügige Öffnung mit 70 cm Innendurchmesser bietet Platz für Patienten gleich welcher Körpergröße, Statur oder Erkrankung. Dank des besonders kurzen Magneten können viele Untersuchungen bei voller Ausnutzung des 45 cm-Messfeldes (FoV) so durchgeführt werden, dass sich der Kopf des Patienten außerhalb der Öffnung befindet. Durch die neu entwickelte ultra-hohe Empfangsspulendichte werden eine höhere räumliche und zeitliche Auflösung sowie ein Untersuchungsbereich von bis zu 205 cm ohne Umlagern des Patienten möglich. Bis zu 204 Spulenelemente erzeugen nun höhere Signalstärken als je zuvor. Dies ermöglicht die flexibelste Bildgebung und unterstützt die anspruchsvollsten Anwendungen.

Ab November 2015 steht diese Technik nicht nur den Spielern des FC Bayern München sondern allen unseren Patienten an unserem neuen Standort in der Säbener Str. 57 zur Verfügung.

Das MAGNETOM Aera verfügt über eine weite 70-cm-Öffnung und ist deshalb auch für Patienten mit Platzangst geeignet, neue Spulen- und Mehrkanaltechnologie und aktuelle Sequenztechnik. Diese Technologie bietet neue Kontrastmechanismen, schnellere Untersuchungen, höhere Auflösung, ist weniger anfällig für Bewegungsartefakte und deckt alle Körperbereiche ab.

CT und MRT zur Untersuchung des Herzens werden derzeit regelmäßig im klinischen Alltag eingesetzt. Dabei haben beide Untersuchungen sowohl Vorteile als auch Nachteile. „Gerade für Nicht-Spezialisten ist es oft schwierig zu entscheiden, für welche klinischen Fragestellungen die CT oder die MRT ein geeignetes Untersuchungsverfahren darstellen“, so Professor Dr. Roman Fischbach aus Hamburg. Er ist einer der Autoren eines neuen Konsensuspapiers, in dem der Stellenwert beider Verfahren gemeinsam von Radiologen und Kardiologen beurteilt wird. Die Bewertungen der Indikationen auf einer fünfstufigen Skala reichen von „zuverlässig einsetzbar“ über „Indikation nur in Einzelfällen gegeben“ bis „keine Indikation“. Die CT erhielt sechsmal die höchste Bewertung, z.B. für Lungenembolie und Beurteilung der Lage eines Schrittmachers bzw. der ICD-Elektroden. Für die MRT gab es 33-mal die höchste Bewertung, z.B. für Myokarditis und Kardiomyopathie.
